Handout: Sample Day One Survey Form

Basic Information and Survey of Web Design Skills

The following is a non-scientific survey intended to tell me something about your aptitudes and skills that relate to designing web pages as part of a team. Please answer honestly. How you respond to these questions will not determine your grade. It will, I'm hoping, give me some information that may help us figure out how you can best contribute to the work we do as a group. Circle the best response to each question.

Complete this form and email it to me as a Word attachment.

You may add additional information about your computer skills at the end of this survey.
