As of January 1, 2007, the Bellingham School District will have all workers' compensation claims handled by the State Department of Labor and Industries.

An Occupational Health & Accident Prevention binder is available at each work site.  Binder contents outline safety, return to work and accident reporting processes.

In the event of a work related injury, ask for the following items from your building secretary or immediate supervisor: 

  • Injured Worker Instructions
  • Employee Injury Report - completed by anyone injured on the job. The immediate supervisor must conduct an independent investigation of the accident and complete the section on the back of the form.  Send the form to Denise Suess, Risk Manager, Central Services or FAX to 647-6886.


If medical treatment is sought, in addition to the above two items, take the following form to your medical provider:

  • Return to Work Release - completed by the medical provider and mailed to Denise Suess, 1306 Dupont Street, Bellingham, WA 98225 or FAXED to Denise Suess at 647-6886.


The Return to Work Release document must be received and reviewed prior to the employee returning to work.
