The absence permit form listed below is in MS Excel format. When you click on the form, it will open in Internet
Explorer as an editable Excel spreadsheet.
This permit is to be used by both certificated and classified personnel for all absences except personal
leave. If three or more employees are attending the same meeting, a roster of the employees may be used instead
of numerous absence permits. The roster must include the same pertinent information: meeting, dates, place,
account number(s) and appropriate signature(s). For individual absences, the employee absence permit must
be completed. When there is a substitute, an account number must be included except when an outside organization
is to be billed for the cost of the substitute in which case you must indicate the name and address of the
organization to be billed.
Steps:
- Employee completes absence permit and submits it to the building/program secretary
- Principal signs to approve absence and account number if building dollars.
- Permit is submitted to Payroll
- Payroll will submit permit to Human Resources if it is a request for emergency/unpaid leave.
The absence permit form is not transferred electronically, you will need to print it and sign it and submit
it to your supervisor for approval.
