• Bellingham High School--Room 212-Mrs. Wilmot, May 6, 2008, 2:30-3:00 p.m
• Squalicum High School--Room A 227-Mrs. Diener, May 7, 2008, 2:30-3:00 p.m.

Submit Health Challenge Registration Form for Health Course Waiver to the school Health Challenge Teacher

November 15, 2008

Take the Comprehensive Health Examination, 9:00-noon, Bellingham High School

March 6 -Grades 9, 10, 11 

Submit all performance task documentation:

• Safe Living and Emergency Response
  Adult CPR and/or First Aid Card

• Consumer Health
  • Community Service:
• SHOC Student Record

or

• Informational Interview:
  • Completed Informational Interview Questions
  • Copy of Thank You Letter
  • Completed Community Partner Evaluation Form
  • Completed Student Reflection Sheet

   

• Social Health, Physical Health, Mental Health and Nutrition Individual Health Risk Analysis and Personal Health Plan

March 31- Grades 9, 10, 11

Notification of successful completion to:

• Student/Parent
• School counselor
• Building principal
• Deputy Superintendent
